Ultrasound Podcast (Five Minute Sono)

In this video I'm gonna do a much-needed revamp of the kidney stone (aka ureterolithiasis) 5 Minute Sono video that I first published back in 2015! In this video I give you some updated information and some higher quality clips.

The way we typically identify the presence of kidney stones using ultrasound is to have a pretest suspicion of kidney stones and then identifying the presence or absence of hydronephrosis. We are typically much more accurate at identifying the secondary findings of an obstructive kidney stone (i.e., hydronephrosis) then we are of identifying the kidney stone itself (but definitely is still possible)!

I am so happy to share with you our latest video on how to perform an ultrasound guided pericardiocentesis! This video has gone through a few revisions but I think this one is my best one so far (although I do concede that I may be a little bit biasedπŸ™ƒ) this video already assumes that you know how to tell a difference between a pericardial effusion and pericardial tamponade. This week on the podcast we have a new version of the 5 Minute Sono video on the sonographic evaluation of the IVC.  The IVC is not going to primarily diagnose anything (usually!), but instead is used to evaluate the central venous pressure (CVP). If the IVC is plump and not collapsing, it means a high CVP, and if it is small and collapsing a lot, it means low CVP.  This is most helpful when evaluating a patient in shock, as it can help push you towards or away from specific diagnoses (such as septic shock vs cardiogenic shock). 

This is actually the third revision of this educational content! First one was in 2015 and the second revision was in 2021.  I’m humbled to have been creating medical education content for as long as I have and I try and keep my videos fresh and up to date :)

In this intro video, we demonstrate the use of ultrasound imaging to identify fractures. Ultrasound-guided fracture identification is a minimally invasive technique that allows for quick and accurate diagnosis of fractures, without the need for radiation or contrast agents. Additionally, it can be used during reductions to assist in identification of adequate alignment without the need for a C-arm!

Also, if you happen to be using a hematoma block as your analgesic, ultrasound can be vastly useful in a precision injection directly into that fracture.

Achilles tendon rupturescan be difficult to diagnose via the physical exam. #POCUS has been shown to have a +LR = 74, -LR of 0.05, a sensitivity of 95% and a specificity of 99% in the diagnosis of achilles tendon ruptures! (This is according to a SR/MA of 15 studies and 808 patients.PMID:34801318).
